Ordering Medications

THERE ARE THREE SIMPLE OPTIONS TO GET YOUR PRESCRIPTIONS THROUGH THE MAIL.

STEP 1 Simply enroll online at www.optumrx.com and follow the simple prompts to place your order. 

STEP 2 Simply complete the Mail Order Service Form  and MAIL the order form to OptumRx, P.O. Box 509075, San Diego, CA 92150-9075 or FAX the order form to (800) 491-7997 (Click Here for Mail Order Service Form)

STEP 3 Simply call OptumRx Member Services toll free at 
1 (800) 797-9791. 

Sending in the Original Prescription

STEP 1 After you complete your set up, you will need to send in the original prescription to OptumRx by mail with your copay for each drug ordered. 

STEP 2 Send the prescription via mail to OptumRx, P.O. Box 509075, San Diego, CA 92150-9075. 

STEP 3 Once your original prescription and copay are received, your prescription will be sent out to you.

Have Your Physician Order Your Prescription Directly

STEP 1 For faster turnaround, have your physician’s office phone in the prescription to our Mail Order service at OptumRx Member Services at
1 (800) 797-9791. 

STEP 2  Additionally, your physician’s office can also fax in your prescription to OptumRx at 1 (800) 491-7997. 

Mail Order Refills

OPTION 1 Refills can be ordered 24/7 online at www.optumrx.com or by calling 1 (800) 797-9791. 

OPTION 2 You can also refill your prescription by completing the Refill Form included in the package with your original order.
